Alzheimer's Disease Programs Initiative Coordinator

About the Position:
This position is an opportunity to build and execute on an important new program at CASL. In this role, a skilled and compassionate leader will oversee the initiative, screen and identify older adults for Alzheimer’s disease and related dementias (ADRD) and deliver appropriate services. The program supports family caregivers of older adults, and offers family-based therapy, individual counseling and consultation. The Coordinator will conduct ADRD awareness campaigns and contribute to the development of the ADRD website. CASL will also support the right candidate in professional development in the social work field, including, but not limited to, pursuit of
licensure in social work and sponsorship for continuing education. Relocation support is also available for this role.
DUTIES AND RESPONSIBILITIES
  • Delivers evidence-based program(s) to family caregivers of the older adults with Alzheimer’s disease and related dementias (ADRD)
o Develops an annual training plan to identify, recruit and offer Savvy Caregiver Program to the caregivers
o Publicizes and reaches out to the community partners and provides training to the identified caregivers
o Conducts pre-, post and follow-up assessments to measure the training
outcomes
  • Offers family-based therapy and individual counseling to families of older adults with ADRD
o Identifies and recruits families of older adults with ADRD to the family-based therapy and individual counseling
o Performs needs assessment, develops goals and treatment plans with identified families, conducts regular evaluation, and plans for termination
o Enters counseling contracts with identified families
o Maintains well documented case notes and counseling records for each of the clients/families
  • Offers consultation to the families of older adults with ADRD
o Identifies families of older adults with ADRD
o Offers consultation to the families using person-centered planning and decision making
o Maintains well documented case notes for each of the families
  • Screens and identifies older adults for ADRD
o Uses standardized assessment tools to screen older adults for ADRD
o Refers clients to healthcare professionals for diagnosis
o Facilitates the care coordination with healthcare professionals
  • Conducts ADRD awareness campaign
o Researches on ADRD and develops informational materials
o Works closely with the agency Graphic Designer to develop and design culturally and linguistically competent materials for marketing, promotion and education purposes
o Sends out ADRD media campaign materials and outreaches to community
partners so that community partners will be made aware of the ADRD services provided by the agency and help identify older adults with ADRD for services
o Plans and schedules ADRD public awareness campaigns twice annually
  • Contributes to the development of a ADRD website
o Leads and works closely with the Alzheimer’s Program to develop culturally and linguistically competent information for the ADRD website
o Collects feedbacks from older adults and the families on ADRD information which can be put on the ADRD website
o Provides feedbacks to the web developer on the website needs and design
  • Supervises Geriatric Workforce Training Specialist and/or other Alzheimer’s Program teammates as assigned, and leads the Alzheimer’s Program to achieve the program objectives, goals and outcomes
  • Prepares regular quantitative, descriptive and analytical reports for management and funder(s)’ review
  • Facilitates interdisciplinary care team meetings
  • Develops and maintains relationships with the community partners on the Alzheimer’s Program
  • Leads, participates in and contributes to Team Meetings, Training Sessions, and Department Meetings
  • Be responsible for own professional development and maintaining valid license in social work and other required certificate as assigned, including participating in the supervisory process and using other educational opportunities, both within and outside of the agency
  • Be responsible for data management agency-wide case management systems (powered by Salesforce)
Education/Experience:
  • Master’s degree in Social Work preferred; Bachelor degree in Social Work as minimum
  • License in Social Work or Clinical Social Work is a plus, but CASL will support the right candidate in securing a license within six months of hire
  • Previous experience in social work, geriatric, counseling or related fields preferred.
Skills/Knowledge:
  • Spoken and written knowledge of Chinese and English (Cantonese preferred, Toishanese and Mandarin a plus)
  • Excellent communication skills, both written and verbal
  • Detail-oriented, with quality and precision-focused
